Annual nursing assessments are required for all people receiving Supported Living. Nursing assessments are required to determine the appropriate level of nursing and other supports needed within Supported Living. There is 24/7 nursing support available. RN's train direct support staff on individual specific plans. They make nursing visits to the individual residence. |

Nutrition |
Nutritional Counseling Services (NCS) is a part of the Supported Living package. This allows for the |
collaboration, consultation, evaluation, assessment, planning, development, implementation, teaching |
and monitoring of a nutritional plan that supports the individual with nutiritional needs to attain or |
maintain the highest possible level of health. |

Monitoring and Supervision |
- Provide and document monthly face-to-face consultation in the Supported Living home, conducted
|
by agency supervisors or Internal Service Coordinators with DSP and the person receiving services |
on at least a monthly basis to include: |
- reviewing implementation of the person's ISP, Outcomes, Action Plans and associated support
|
plans, including HCPs, MERPs, PBSP, CARMP, WDSI; |
- scheduling of activities and appointments and advising the DSP regarding expectations and next
|
steps, including the need for IST or retraining from a nurse, nutritionist, therapists or BSC; and |
- assisting with resolution of service or support issues raised by the DSP or observed by the
|
supervisor, Service Coordinator, or other IDT members. |
- Monitor the DSP implement and document progress of the AT inventory, physician and nurse
|
practitioner orders, therapy, HCPs, PBSP, BCIP, MERPs, CARMPs. |
- Monitor and document monthly that the devices listed in the WDSIs are available, functioning
|
properly, and are used and communicate issues related to AT devices to the appropriate therapy |
consultant or IDT member. |
